A doctor's surgery has been rated inadequate by the Care Quality Commission.

Dr Shantir's Practice at Forest Road Medical Centre in Forest Road, Waltham Forest, was rated inadequate overall by the healthcare regulator after an inspection in November.

It was ranked inadequate for being safe, responsive and well-led and rated as requires improvement for being effective and caring.

Inspectors found that safeguarding systems were unclear and did not keep patients safe or protected from abuse.

The practice did not have monitoring systems for managing outcomes and actions associated with risk assessments such as infection prevention.

The vaccine refrigerator temperature was also found to be ineffectively monitored.

The surgery was found not to monitor or manage inadequate smear rates.

It was additionally discovered that personnel records for some clinical members of staff including training records, professional indemnity and professional registration status.

The practice had previously received a requires improvement rating in 2017.

It was found that insufficient improvement had been made since the Care Quality Commission's last inspection.

Professor Ursula Gallagher, deputy chief inspector at Primary Medical Services, said: "I am disappointed that care at Dr Dauod Yosuf Abdulrahman Shantir’s practice has deteriorated since we last inspected. We have now placed it in special measures. Special measures will give people who use the service the reassurance that the care they get should improve."
